DEFINITIONS
In this Agreement, the term "goalkeeper training programs" shall include all activities, camps, clinics, programs, events, classes, and services provided, sponsored or organized by Last Line of Defence Goalkeeper Academy including but not limited to: goalkeeper training; orientation or instructional sessions or lessons; weight training; personal training; use of strength training and fitness conditioning equipment, machines and facilities; nutritional and dietary programs; merchandise recommendations; and all other such related activities.

ASSUMPTION OF RISKS
I am aware that participation in goalkeeper training programs, involves risks, dangers and hazards, which could result in damage, loss or physical injury to me. Some of these risks, dangers and hazards include, but are not limited to:

• Health: overexertion, dehydration, fatigue, lack of fitness or conditioning.
• Premises: defective, dangerous or unsafe condition of facilities; falls; collisions with objects, equipment or persons.
• Use of Equipment: mechanical failure of the equipment; negligent design or manufacture of the equipment; the provision of or the failure by the Releasees to provide any warnings, directions, instructions or guidance as to the use of the equipment; failure to use or operate the equipment within my own ability.
• Advice: negligent advise regarding goalkeeper training programs and fitness programs.
• My conduct and conduct of other persons, including NEGLIGENCE ON THE PART OF THE RELEASEES, may increase the risk of damage, loss, personal injury or death. I understand that the Releasees may fail to safeguard or protect me from the risks, dangers and hazards of the goalkeeper training programs, some of which are referred to above.

Despite the risks, dangers and hazards of goalkeeper training programs, and fully understanding such risks, dangers and hazards, I WISH TO PARTICIPATE IN GOALKEEPER TRAINING PROGRAMS WITH LAST LINE OF DEFENCE GOALKEEPER ACADEMY, AND I FREELY ACCEPT AND FULLY ASSUME ALL SUCH RISKS, DANGERS AND HAZARDS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, PROPERTY DAMAGE AND LOSS RESULTING THEREFROM. EMERGENCY FIRST AID.
Should I be injured, I give permission to Last Line of Defence Goalkeeper Academy to provide emergency medical treatment. I acknowledge and agree that all expenses associated with emergency first aid will be my responsibility and will not be covered by the Releasees. I agree that Last Line of Defence Goalkeeper Academy, nor its Releasees, shall not be held responsible for any liability, injury or accident during, or traveling to or from, any goalkeeper training programs.

RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT
In consideration of the Releasees allowing me to participate in goalkeeper training programs and permitting my use of their equipment, facilities and services as defined in this Release Agreement, and for other good and valuable consideration, the receipt and sufficiency of which is acknowledged, I hereby agree as follows:

1. TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have against the Releasees and TO RELEASE THE RELEASEES from any and all liability for any loss, damage, expense or injury including death that I may suffer, or that my next of kin may suffer as a result of my participation in goalkeeper training programs, due to any cause whatsoever, including negligence, breach of contract, or breach of any statutory or other duty of care, including any duty of care owed under the Occupiers' Liability Act, R.S.B.C. 1996, c. 337, on the part of the Releasees, and further including the failure on the part of the Releasees to safeguard or protect me from the risks, dangers and hazards of goalkeeper training programs referred to above;
2. TO HOLD HARMLESS AND INDEMNIFY THE RELEASEES from any and all liability for any property damage or personal injury to any third party resulting from my participation in goalkeeper training programs;
3. This Agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives, in the event of my death or incapacity;
4. This Agreement and any rights, duties and obligations as between the parties to this Agreement shall be governed by and interpreted solely in accordance with the laws of the Province of Alberta and no other jurisdiction and I agree to attorn solely to the jurisdiction of the Court of the Province of Alberta; and
5. Any litigation involving the parties to this Agreement shall be brought solely within the Province of Alberta and shall be within the exclusive jurisdiction of the Court of the Province of Alberta.

INSURANCE
I am aware that the Releasees do not provide me with any disability, accident, liability or medical insurance or compensation, should I become injured or cause personal injury or property damage to any third party while participating in goalkeeper training programs.

PRIVACY
I agree to have my personal information, as provided for registration purposes, distributed as necessary by Last Line of Defence Goalkeeper Academy staff and agree to have my identity published in print, electronic, or digital format.

CANCELLATIONS, REMOVALS AND ASSIGNMENTS
I agree to Last Line of Defence Goalkeeper Academy reserving the right to assess and assign me to the appropriate goalkeeping training level as assessed by their staff; reserves the right to request any participant withdraw from the Academy if the participant is not acting in an appropriate and responsible manner; and to cancel any goalkeeper training program with 100% refund. VIDEO / PHOTOGRAPHY RELEASE I grant the Releasees the non-exclusive right to photograph me and make audio-visual recordings of me for use in educational, marketing and advertising purposes. In entering into this Agreement I am not relying on any oral or written representations or statements made by the Releasees with respect to the safety of goalkeeper training program, other than what is set forth in this Agreement.

I CONFIRM THAT I HAVE READ AND UNDERSTOOD THIS AGREEMENT PRIOR TO SIGNING IT, AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS, ASSIGNS AND REPRESENTATIVES MAY HAVE AGAINST THE RELEASEES.
